The form here is provided for accepted Guest Artists to complete their booth payment and provide Fair program information.
Please be sure that you are selecting the booth size in your placement email. If you were assigned a corner placement, be sure to select that additional fee. You should review the electricity that is provided and select additional electricity as needed.
You must sign digitally to agree to the terms of our Guest Booth Holder contract, available for reference HERE.

When you submit, please click the submission button ONLY ONCE. You should see a message on your screen once the application submits, followed by an email confirmation containing a copy of your payment information and submitted information. If you do not see that message and do not receive the follow-up email, then your submission did NOT go through. There may be an error message somewhere in the form that requires your attention.
